Introduction

Alzo Health is an AI-based SaaS platform for tracking memory performance and assessing cognitive health. At Alzo Health we employ network science and AI to track memory performance and assess cognitive health, generating personalized insights through complex network graphs. Utilizing AI-analysis, we developed special family network graphs and a unique Spectra Score for each person to see how their memory is doing. Also, Alzo Health utilizes AI assistance to inform users about memory decline risk and offer guidance. With an AI Doctor, we provide expert advice for interpreting cognitive health data and suggesting next steps, making it a one-stop solution for cognitive health management and memory tracking.

These are the terms and conditions ("Terms of Use") that govern your usage of our websites, including https://alzo.health/, (collectively, the "Sites"), and our app or software Alzo Health. By visiting the Sites, you accept these Terms of Use. Your access and use of our Services and Platforms (App, Sites ) implies your acknowledgment that you have read, understood, and consented to be bound by these Terms and our Privacy Policy. If you disagree with or do not consent to these Terms or our Privacy Policy, you are not authorized to use our entire range of services, including our site, app, software, the HIPAA compliance onboarding form, and accounts.

Accounts

To use the Alzo Health platform and services, you must have an account. Keep your password secure and robust, as you are responsible for all activities associated with your account.

• 1. Eligibility: You must be at least 18 years of age to use our Services. By agreeing to these Terms and creating an account on our sites, if you are a caregiver, you certify that you are a family member or caregiver who has known the patient for at least 5 years or more.
• 2. Account Security: Your account is personal to you , and you may not share your account information with any third party or allow unauthorized access.
• 3. Account Information: You are responsible for providing accurate and complete information when setting up and maintaining your account. You must furnish valid email addresses, information regarding your and relationship with the patient. . You have complete responsibility for your account and the information entered in our onboarding form on behalf of the patient.
• 4. Lawful Use: You may not access or use the Services or create an account for unlawful purposes or on behalf of a patient you do not know. Your use of the Services and behavior on our platform must comply with applicable laws and regulations. You are solely responsible for understanding and complying with the laws and regulations applicable to you concerning the accuracy of the information you enter on our site and onboarding forms on behalf of the patient.

Privacy

Our Privacy Policy applies to your use of the Sites, and its terms are incorporated into these Terms of Use.

Restrictions, Ownership, and License

All rights, title, and interest (including patents, copyrights, trademarks, and other intellectual property rights) in our app/software, sites, and materials belong to Alzo Health or the original creators. All games, activities, names, designs, graphics, data, images, logos, and icons are proprietary and the exclusive property of Alzo Health or the original creators, protected by U.S. copyright law. You are not permitted to modify, copy, distribute, transmit, display, perform, reproduce, publish, license, create derivative works from, transfer, or sell any information obtained directly from the Services and Alzo Health platforms

Alzo Health reserves the right to monitor, edit, or remove any information entered by caregivers or family members on behalf of patients. We assume no responsibility or liability for any content entered in our onboarding forms, and we trust that caregivers and family members provide accurate information in our Sites and platforms, especially in our HIPAA-compliant onboarding form.

Subscription Terms

By using a Subscription Plan, you agree to the additional terms as follows:

• 1. Subscription Cancellation: You may cancel your subscription at any time. If you cancel your subscription, it will automatically end on the last day of your billing period. Cancellation does not terminate your Alzo Health account or the data you entered in our platform's onboarding form and site. Alzo Health reserves the right to modify, suspend, or terminate the Subscription plan, access, or operation without notice under specific circumstances. • 2. Payments and Billing: The billing cycle is the first day of every month. A failure to pay will terminate the subscription plan and any other services provided through the Alzo Health Platform. Notices regarding payment failure will be communicated to the account holder.
• 3. Limitation of Liability: Alzo Health cannot guarantee improvements in patient conditions through our platform and services. It is designed to assist to monitor memory performance. We also cannot guarantee complete protection against hacking or technical issues that may affect the site, app, or services. Usage and any changes to the subscription plan and pricing will be communicated to users in advance
